Why Turnarounds (TARs) deserve our full attention
When we think about safety in the process industry, our focus is often on normal plant operations. However, experience and industry statistics consistently show that many serious incidents occur during maintenance shutdowns, Turnarounds (TARs), and plant outages.
Why?
Because during a TAR, everything changes.
Equipment is opened, pipelines are disconnected, vessels are cleaned, temporary systems are installed, and multiple contractors work alongside operators at the same time. Normal operating conditions no longer exist, making planning, communication, and collaboration more critical than ever.
It often starts with the small things.
A heat exchanger needs to be cleaned.
A storage tank appears to be empty.
A pump has been isolated.
A pipeline has been flushed.
At first glance, these seem like routine maintenance activities. However, hidden risks can remain if equipment still contains residues of flammable oils, hydrocarbons, or vapors. A single spark from hot work, the use of the wrong tool, or an inadequately reviewed work permit can be enough to trigger a serious incident.
More often than not, accidents are not caused by one major technical failure. They result from a chain of small assumptions.
"It should already be clean."
"The operator has probably checked that."
"That must have been covered during the work preparation."
And that is exactly where the danger begins.
Maintenance activities are often underestimated.
Maintenance work is sometimes viewed as routine or less critical than production activities. In reality, maintenance introduces some of the highest risks because the plant is operating outside its normal conditions.
- Work scopes change.
- New contractors arrive on site.
- Work permits are updated.
- Schedules shift.
- Tasks are handed over between different teams and shifts.
When communication is incomplete or assumptions replace verification, the likelihood of incidents increases significantly.
Good work preparation is the foundation of safety
Effective work preparation is much more than creating a schedule.
It means ensuring that everyone clearly understands:
- What hazards are present;
- Which energy sources have been isolated;
- Whether equipment has been thoroughly cleaned, depressurized, and gas tested;
- Which personal protective equipment (PPE) is required;
- Which simultaneous operations may introduce additional risks;
- Who is responsible for each activity.
Procedures are essential, but procedures alone do not prevent incidents. People do.
Never stop asking questions
One challenge I continue to observe in industry is that communication is sometimes seen as unnecessary, repetitive, or even inconvenient.
Yet, the questions that are asked one more time are often the questions that prevent an accident.
- Has the equipment really been cleaned?
- Has the isolation been independently verified?
- Do operators and contractors have the same understanding of the work scope?
- Has everyone been informed of the latest changes?
A safe workplace is built on a culture where people feel comfortable asking questions and challenging assumptions.
Safety is a shared responsibility
A turn-around is arguably one of the most critical periods in the lifecycle of a processing facility. Not only because of the volume of work taking place, but because success depends on close collaboration between operators, maintenance personnel, engineers, supervisors, and contractors.
Safety does not happen by chance.
It is achieved through careful preparation, clear communication, and a willingness to replace assumptions with verification.
Because ultimately, safe maintenance does not begin with a work permit.
It begins with a conversation.
The most dangerous moment in a plant is often not when it is producing; it is when people believe the process is no longer dangerous because it has been shut down for maintenance.
